Step by step directions to renew certificate:
Request Renewal Certificate and CEU Verification Forms from Anita Sams.
Submit the following to Anita Sams:
A. CEU Verification Form with proof of completion of 4 semester hours or 12
CEUπs completed since current certificate was issued.
B. Completed application
C. Check made payable to Ohio TEC (amount on application form)
ESC will submit the LPDC approval form and completed application to the state
for processing.
NOTE: Only 8-year certificates issued prior to September 1, 1998 are eligible to be renewed. All others must transition to license.
Step by step directions: how to transition to license
1. Request Transition to License application from Anita Sams or download from ODE.
2. Submit the following to Anita Sams:
A. Proof of 18 CEUπs OR 9 quarter hours, OR 6 semester hours OR a combination completed since current
certificate or license was issued.
B. Completed application form
C. Check made payable to Ohio T.E.C. (amount is indicated on application)
3. E.S.C. will submit your application to the state and send you our copy of your license when it is received.
