Procedures

Q:  I've had an injury on the job.  What should I do?

A:  Contact Mary Lou Shafer (x116) or Anita Sams (x105) at
the Greene ESC - (937) 767-1303.

If it is a serious injury - report to Urgent Care or the Emergency Room, 
and provide this Policy# 32900051-0.

 

If it is not an emergency, report to Greene Memorial Hospital Work Plus,

                                                              phone 458-4220
                                                              hours 8:00-4:30pm
                                                              Give the ESC policy # 32900051-0

 

Then call Mary Lou or Anita and report the follow up information regarding your injury.

Turn in an Employee Accident Report to your supervisor and to Mary Lou Shafer.
